Health Possible (“Health Possible”) (“We”) (“Our”) reserves the right to edit these Guidelines at any time and without notice. We regularly edit and improve upon our Guidelines based on the real life, logistical, and practical experiences of our programs and recommend checking them regularly during the time of your active time with us. These Guidelines are a reference for our applicants, alumni, members, and staff alike. All parties involved have the most optimal experience with and greatest understanding of Health Possible’s program when the Guidelines are reviewed and leveraged often and appropriately. Please review all of the following sections carefully:

Section 1.0 Application Process

  1. Visit http://www.healthpossibleinc.org
  2. Click “Apply”
  3. Enter the applicant’s income as accurately as possible; adding up paystubs to the hundredth decimal place will provide the most accurate financial aid results.
  4. Upload proof of the applican’ts most recent 90 days worth of income – this means 90 days backwards starting from the date in which you are applying. If applicant does not have income as recent as same-day, then upload income as recent as your most recent pay’s end-date, backwards (i.e. If pay was for January 1st – January 14th, the end-date is the 14th, and 90 days backwards from January 14th is October 14th of the prior year). Health Possible uses these bank statements, paystubs, etc. supporting documents to verify that the applicant entered their income correctly, as required per #3 of this list.
  5. Upload proof of the applicant’s most recent Federal Tax Return. Health Possible requires proof of the applicant’s most recent tax return to verify that the applicant does not have additional or undisclosed income not initially accounted for in #3.
  6. Double check that all required documentation is uploaded and “Attached” or “Added” to the application. A lack of legal and legitimate documentation can result in a delayed verification and enrollment process, or complete declination in Health Possible programs. Health Possible reserves the right to decline applicants with no re-application if the staff has ANY reason to believe an applicant is being deceitful or fraudulent about his/her income, or engaging in illegal activities for monetary gain. Health Possible does not support abusive or illegal practices for employment or monetary gain, and reserves it’s financial aid for the ought-most deserving applicants.
  7. Submit the Application
  8. Check primary email and spam inboxes for Health Possible’s automated emails, explaining next steps of the application process.

Section 2.0 Health History & Legal Acknowledgements

  1. Once accepted to Health Possible for Wellness Financial Aid (WFA) (also known as our Scholarship), the user (registered email) will receive an automated email with a link to log back into the account.
  2. Once logged in, the user will be re-directed to complete the applicant’s Health History Questionnaire and carefully review all legal acknowledgements, which is an all-in-one required form to participate in a health and wellness program financially endorsed by Health Possible.
  3. Double check that all required documentation is filled out accurately and to the best of the applicant’s knowledge. False or undisclosed information or data as required by the form can result in a delayed enrollment process, or complete declination in Health Possible programs. Health Possible reserves the right to decline applicants with no re-application if the staff has ANY reason to believe an applicant is being deceitful or phony about his/her health data or information, or placing themselves or others in harms way by trying to participate in a health and wellness program that could be dangerous to his/her health. Health Possible does not support immoral or deceitful practices of any kind, and reserves it’s financial aid for the ought-most deserving applicants.
  4. Sign and submit the forms.
  5. The applicant will be contacted by a representative within 24 hours. If a representative does not contact the applicant within 24 hours, we encourage the applicant to call or text Health Possible regarding his/her completed Health History Questionnaire at (910) 800-0287 or by email at info@healthpossibleinc.org

IMPORTANT NOTES BEFORE FINALIZATION:

  • All enrolling applicants receive financial aid for a maximum of six months in length.
  • All enrolling applicants and complying members are budgeted to receive a maximum of $200 per month in Health Possible financial aid.
  • Health Possible limits its financial aid for the purchasing of fitness, nutrition, mental health care, naturopathic, and alternative medicine services on behalf of its enrolling applicants.
  • Health Possible refuses to pre-purchase care on behalf of an applicant or member for MORE THAN $200 per month, unless a special, designated, and potentially revitalizing donation is pre-made specifically on behalf of or in the name of the applicant or member.
  • We work hard to plan a sustainable lifestyle/program for applicants during and AFTER the sixth month that he/she has community within and can afford to continually participate in, without financial aid.
  • All applicants are required to join Health Possible’s Alumni & Member’s brand new online Meetup Group here (click link). This Meetup will host free and pay-anything events that you can participate in for life, and it’s ONLY for program beneficiaries!

Section 3.0 Care Coordination

  1. The applicant should already have an idea of the health and wellness care, membership, appointments, program, or plan he/she would like to participate in.
  2. If not, the applicant should begin searching online health and wellness marketplaces such as, but not limited to (click on any): Upper, Kensho, Heal.me, Ness (acquired WellSet), ClassPass, and Sesame (for wellness purposes only).
  3. The applicant can also search for health and wellness using any online search engine (i.e. Google, etc.) or any specific provider/businesses direct website (i.e. http://www.”alocalgym”.com)
  4. If the applicant knows exactly what he/she would like to participate and enroll in, the Health Possible representative should contact the applicant within 24 hours and, once given the exact name, address, phone number, email, and point of contact for the business, our staff can begin a business-to-business price negotiation and payment schedule with the care provider.
  5. If the applicant does not know exactly what he/she would like to participate in, the applicant will default into the Health Possible structured membership program, which includes one fitness program, and/or one nutrition program, and/or one mental health program, and must equate to $200 per month or less in total costs. Should the applicant default to a Health Possible membership, he/she will need to undergo a Care Coordination Call to determine provider(s) for each service.
  6. A Health Possible representative should contact the applicant within 24 hours of all-document-completion to schedule a 30 – 60 minute Care Coordination Call, which will be based on the Health History Questionnaire data and information provided.
  7. During the call, the applicant and the Health Possible representative will collaboratively search for and decide on the most optimal health and wellness solution(s). Each applicant is only provided with one collaborative phone call to ensure our staff can serve every applicant with ample time. All applicants need to do as much research as possible on his/her own time prior to a scheduled call and come prepared.
  8. If by the end of one Care Coordination Call, an applicant still is not confident about his/her needs or options, the Health Possible representative will recommend a Medical Evaluation by a Naturopathic Doctor (ND/NMD) or a Doctor of Osteopathic Medicine (DO), and will request Orders from the Physician for care coordination continuance. Medical Evaluations by NDs, NMDs, and DO’s are paid for in compliance with the applicants first month of awarded financial aid. Money leftover after the paid-for evaluation are what is available for program care.
  9. If the applicant refuses a Medical Evaluation, he/she has 48 hours to email their final care choice(s) to their designated representative, which they are left to determine without further assistance. If the applicant fails to email the representative by the 48th hour, he/she automatically forfeits his/her enrollment in its entirety to, and any financial aid from, Health Possible. Applicants who forfeit their enrollment by ‘failure to complete the care coordination process in a timely manner’ are eligible to re-apply beginning 90 days after their most recent (current) application date (all new income and health documentation will be required). Applicants eligible for re-application who re-apply after 90 days will be required to know their preferred care immediately upon acceptance. Re-application after a second attempt (a third application) is not allowed and will be discarded.

Section 4.0 Enrollment

An applicant is not considered “active” nor is an “active member” of Health Possible until providers are pre-paid for care and the applicant successfully attends and completes his/her first appointment.

Health Possible will do its best to negotiate costs with all providers who are not available in an online marketplace (where rates are already available for lower than their market rates).

Health Possible will aim to pre-purchase at least 30 days worth of care on behalf of the applicant via the providers website or by phone. Health Possible purchases care by credit card or direct deposit only.

The applicant will be notified by text or email when his/her care is officially paid for and he/she can officially begin scheduling and attending appointments.

Section 5.0. Payment

Per our Honor System, we anticipate that every applicant will gladly make a voluntary donation to Health Possible for the amount he/she did not qualify for in financial aid. For example, if an applicant qualified for 50% in financial aid and their program cost is $100 that month, Health Possible would expect the applicant to voluntarily donate $50 by the 31st day of that same month. Note, while no donations are ever required, Health Possible has faith that its applicants would gladly give to an organization that is giving to them. Your voluntary donations allow us to serve more people, and on a deep level, in similar situations as you.

Health Possible kindly asks all of its active members if they would like to make a donation in the amount of their unqualified amounts once or twice per month. We appreciate when any of our members give back to our program.

Note: Health Possible cannot accept any other method of payment that is not considered a voluntary donation, requested in kindness and good faith, such as but not limited to: member bill reimbursements, bill splitting with members, etcetera models, in order to refrain from “acting like an insurance company.”

Health Possible is not, nor should ever be thought of, construed as, referred to, or is anything like an insurance company. Health Possible is not insurance; Health Possible is a 501(c)(3) charitable Nonprofit Corporation that pre-pays for 100% of the costs of health and wellness services on behalf of low-income and qualifying applicants. We hope all beneficiaries of our financial aid choose to support Health Possible’s efforts in providing them with an opportunity to a more educated, positive, and proactive lifestyle.

Section 6.0. Scheduling & Attendance [IMPORTANT REQUIREMENTS TO MAINTAIN MEMBERSHIP]

  • All applicants must schedule their first appointment within 24 hours of being notified of the completed purchase.
  • All applicants are required to email the confirmed date, time, location, and for what service/with which provider of their first scheduled appointment to their designated Health Possible representative or info@healthpossibleinc.org. Health Possible reserves the right to verify appointment dates and times with the provider(s). Health Possible reserves the right to confirm a Members’ punctuality, overall attendance, and completion of appointments with the provider at random and at any time, with or without reason.
  • All applicants are required to request and complete the Personal Health Inventory (PHI) 24 hours before their first appointment. A Health Possible representative will email the applicant the PHI to digitally complete.
  • All members are required to request and complete the Personal Health Inventory (PHI) exactly half-way through his/her program. A Health Possible representative will email the applicant the PHI to digitally complete.
  • All members are required to request and complete the Personal Health Inventory (PHI) exactly one day after his/her last day of appointments paid for with Health Possible financial aid. A Health Possible representative will email the applicant the PHI to digitally complete.
  • These documents are used to show legitimate member progress to the member most importantly, our staff, and keeping the member anonymous – shown to prospective and active donors and sponsors as a vital fundraising tool.
  • All active members are required to make and keep a schedule of appointments at least 30 days in advance that can be shared with a Health Possible representative randomly and at any time.
  • Applicants and active members may not be tardy to or miss more than two appointments of any kind throughout their six month program. A third tardy or miss will result in automatic termination of membership and financial aid from Health Possible.
  • Health Possible appreciates any donations from the member to recover any of the terminated member’s non-refundable losses for future pre-paid care to assist the next member with. Preferably, a donation in the full amount of the nonprofits losses on future pre-paid care is greatly appreciated. Members can contact their designated representative to request the total un-used balances if he/she would like to make a donation.
